Job Description

Under the direction of the Community Health Response (CHR) Supervisor, the Community Health Responder Team Lead is a field-based position that works hands-on with members of the community to provide service related to COVID-19 testing, case investigation/contact tracing, vaccination, outreach, and education. CHRs will work closely with CBOs, Fulton County Board of Health epidemiology team, GSU, and other agencies to bring awareness and provide services related to COVID-19 testing, isolation/quarantine guidance, case investigation, and vaccination.
We require all employees to comply with our Mandatory Vaccination Policy. As a condition of employment you will be asked to provide proof of COVID-19 vaccination.Responsibilities:
  • Convey the purpose, outreach services, and impact of the program to the priority population.
  • Promote COVID-19 prevention measures such as social distancing, wearing a well-fitting mask, etc.
  • Screen for COVID-19 symptoms while at the doorstep. Offer rapid antigen tests for symptomatic individuals and work closely with the team RN to vaccinate individuals with negative test results.
  • Direct and assist interested individuals with the Curogram link to self-register for a COVID-19 vaccine.
  • Document all encounters, complete and submit daily reports, and maintain comprehensive patient communication notes across multiple databases.
  • Assist in field-based humanitarian response activities to support efforts to smother COVID-19 outbreaks.
  • Communicate openly and maintain good working relationships within the team, government and other local authorities, donors, and other stakeholders.
  • Refer eligible individuals to CORE Resource Coordinators for resource provision.
  • Lead and participate in outreach efforts to raise community awareness of services provided.
  • Perform daily inventory, and other organizational tasks as assigned.

The CHR Team Lead will also be responsible for:
  • Providing guidance, in field training, and mentorship that encourages the team to perform at their optimum.
  • Imparting team members with skills to improve confidence, program knowledge, and communication in all field operations.
  • Lead operations for community events when necessary.
  • Conducting weekly inspections to ensure that the team is maintaining all safety protocols for successful program operations.
Qualifications:
  • High school diploma or GED
  • Any combination of 2 years' health/social services experience
  • Experience conducting home visits preferred
  • Must have a valid Driver’s License, verifiably good driving record, and reliable transportation
