Job Description

Under the direction of the Clinician Manager and the Fulton Health Equity program management, the Home Visit Vaccinator is a field-based position that works hands-on with members of the community to provide antigen testing and vaccination services requested by Fulton County residents at the household level. Vaccinators will travel and work with Community Health Responders to visit households of residents requesting vaccinations and/or antigen testing at their residence. Operates out of the 4700 North Point Parkway Alpharetta location but travels with team members throughout Fulton County. Teams may also be asked to respond to shelters and group homes to provide antigen testing and vaccinations.
We require all employees to comply with our Mandatory Vaccination Policy. As a condition of employment individuals will be asked to provide proof of compliance to this policy within 30 days of employment.Responsibilities
  • Collect data pertinent to the healthcare consumer's health or the situation
  • Obtain consent and provide testing and immunization education
  • Performs anterior nasal swab specimen collections for rapid antigen testing
  • Provide post-immunization monitoring and education to ensure the health and safety of each individual
  • Delegate elements of care to appropriate healthcare workers in accordance with any applicable legal or policy parameters or principles
  • Collaborate with healthcare consumer and other key partners in the conduct of nursing and paramedic practice
  • Ensure sanitary environment by overseeing implementation of infection control best practices
  • Contribute to quality nursing and paramedic practice
  • Evaluate one's own and others nursing and paramedic practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations
  • Utilize appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible
  • Practice in an ethical, environmentally safe, and healthy manner
  • Other tasks as needed
  • Proficient in English, additional bilingual ability a plus
Requirements
  • All personnel working on this project must be able to provide documentation verifying that they are fully vaccinated in order to be in compliance with FCBOH requirements
  • Current Georgia LPN/RN or Paramedic license within good standing.
  • Current Basic Life Support certification; Current CPR certification
  • Graduate from an accredited school for applicable medical degree 1+ years clinical experience performing relatable medical care or 2+ years experience as a Paramedic
  • Experience administering COVID-19 vaccinations preferred
  • Proficient with injections and comfortable with all age ranges
  • Reliable team player, who over communicates and takes extreme ownership in her/his work.
  • You will be trusted to work with minimal supervision and we are counting on you to deliver thoughtful, compassionate patient care
  • Comfortable with change. The pandemic has brought about a new meaning to the term flexible.
  • The pandemic is driving where we are needed so you will need to be comfortable with flexible hours and changes in locations
  • Detail oriented to ensure infection control and patient safety
  • Willing to work weekdays, weekends, holidays, as needed; flexibility with availability
  • Reliable transportation and clean driving record
  • Willing to travel to different locations throughout Fulton County.
  • Able to lift up to 25 pounds
